As a Prophetess and a Minister of the gospel of Jesus Christ, this individual has dedicated her life to serving the lord and helping others through the field of case management. With over 25 years of experience working with gerontology, the elderly and individuals with severe disabilities. She has become a trusted figure in the community. For 10 years, she served as an executive assistant at Corners of Care case management before venturing out on her own in 2019. She founded her own company, 360 Case Management, which focuses on providing Medicaid Services programs for Georgia and South Carolina. Her passion for helping others extends beyond just physical health as she holds a career degree in diet nutrition and Health Science from Ashworth College. Additionally, she is a graduate of the Dr. Sears Health and Wellness Institute where she became master certified in health and wellness. With a love for people spiritually first and foremost, she uses her expertise to provide resources, tools, and education for better outcomes in their journey towards health. In today's society, we often see a gap in the healthcare system where certain individuals may fall through the cracks. They may not be elderly or severely disabled, but they are still struggling with health issues that greatly impact their quality of life. Recognizing this need for expanded services, two years ago, she embarked on a new journey to establish 360 Chronic Care Management. This company aims to provide resources, education, and insight to help better manage and potentially even reverse certain diagnoses. As a strong believer in Jesus Christ and His gospel, she is driven by her love for Him and her desire to make a positive impact in the lives of others. She also values spending time with her family and friends, which further motivates her to bring holistic options to those in need of care. Nateka Austin is new to Augusta, GA area relocated from Charlotte, North Carolina. She obtained her BSN from Winston Salem State University in Winston Salem, North Carolina in 2015 and graduated from South University in Savannah, Georgia with her Masters Degree in Nursing for Adult Gerontology Primary Care Nurse Practitioner in December of 2018. Her first RN position was held in Carolina Hospital in Florence, South Carolina, which is currently MUSC-Florence, South Carolina in the area of Medisurg then transitioned to Critical Care ICU and CVICU for 10 years. Her passion for Nursing was also in the community in the field of Long Term Care, Mental Health, and Hospice. The drive pushed her to pursue her lifelong goal of becoming a nurse practitioner. While attending graduate school, she worked full-time at Carolinas Hospital- Main, Charlotte, North Carolina currently Atrium and Presbyterian Hospital, Charlotte, North Carolina currently Novant Health. While in this position, she prides herself on being a mentor and leader and was promoted to Nursing Director for Critical Care Units for 3 years. Throughout graduate school, she found her passion for Nursing and continued to drive to provide the best possible care to her patients as she transitioned to the role of Nurse Practitioner, especially caring for the Geriatric patient population. She is excited to grow and learn from the amazing medical community in Augusta, Georgia. She currently is a Hospitalist Nurse Practitioner at Piedmont in Augusta, Georgia and Hospice Nurse Practitioner for Crescent Hospice in Augusta, Georgia. Currently in school for Post Certification in Mental Health Nurse Practitioner. She wishes to go into Nursing education to promote the growth of Nursing. She has written research articles in Medical journals and participated in Medical Research that focused on Geriatric population related to Chronic Care Management. She enjoys spending time with her family, and friends.

With over 10 years of experience in accounting and bookkeeping, I have had the privilege of working with people from North Carolina, South Carolina, and Georgia. This has allowed me to steadily grow and expand my skills in this field. In 2013, I received my Bachelor of Science in Business Management and Accounting. Throughout my career, I have had the opportunity to work with small businesses across various industries. With over 20 years of experience in case management and program administration, I have had the privilege to work with Medicaid programs in both Georgia state waiver and elderly and disability programs such as CCSP, SOURCE, ICWP, and South Carolina CLTC; as a Program Manager and Senior Case Manager for 360 case management, I am truly blessed by the Lord Jesus to have held this role for the past four years. With an expanded position as program manager for 360 Chronic Care Management, I oversee workflow and administration processes for nurses and case managers who work tirelessly to support our elderly and disabled, and newly added Medicare services. With my degree in administration, with a certification and license as a Massage Therapist, I am able to provide holistic care and education that supports overall well- being for individuals on their journey towards better health. It is my hope to make a positive impact in people's lives through comprehensive care management services.
